  • We are looking for candidates with Board Certification or Board Eligibility in Internal Medicine, Family Medicine, Geriatrics, or a related specialty.
  • A valid State Medical License is required.
  • Commitment to ongoing board certification, including necessary Maintenance of Certification (MOC), Continuing Medical Education (CME), and examinations is essential.
  • Current Basic Life Support (BLS) certification is a must.
  • Possession of a DEA number for controlled substances is necessary.
Position Overview:
  • Provide specialized care for elderly patients with both acute and chronic health issues.
  • Assume full responsibility for patient care and outcomes, working in collaboration with specialists as needed.
  • Work alongside hospitalists for inpatient care management.
  • Deliver assessment, diagnosis, treatment, education, and coordination of care for complex health needs.
  • Lead a multidisciplinary care team, including a medical assistant, care facilitator, and care coordinator.
  • Engage in managing care for patients across various settings, including hospitals, skilled nursing facilities, long-term care facilities, or homebound individuals.
  • Participate actively in center management and assist in patient and provider recruitment efforts.
