Key Responsibilities- Facilitate cognitive interventions and train individuals in functional skills related to personal care, such as toileting, bathing, grooming, dressing, and eating.
- Demonstrate effective communication with clients, their families, and support systems, while maintaining professional boundaries.
- Assist with personal care and ADLs, ensuring hygiene and eating needs are met, and completing daily tasks.
- Train individuals in functional skills related to community resources, such as transportation, shopping, social, and recreational activities.
- Monitor health and physical condition, and assist with medication management, if applicable.
- Recognize and report suspected abuse, neglect, or exploitation.
- Supervise to ensure the health and safety of individuals.
- Facilitate behavioral supports and train individuals in adaptive behavior for home and community.
- Facilitate active involvement of clients and their families in the service delivery process, and maintain knowledge of daily goals and document work on achieving those goals.
- High School Diploma or GED
- Valid driver's license and reliable transportation
- 1 year of experience working with individuals with disabilities (preferred)
- Ability to deal with others using courtesy, tact, and good judgment
