Job Title: Licensed Practical Nurse II

The Illinois Department of Human Services is seeking a Licensed Practical Nurse II to join our team at the Choate Mental Health and Developmental Center in Anna, Illinois.

Job Summary:

We are looking for a skilled and compassionate Licensed Practical Nurse II to provide direct nursing care to individuals with developmental disabilities. The successful candidate will work collaboratively with our interdisciplinary team to ensure the highest level of care and support for our clients.

Responsibilities:
  • Provide direct nursing care to individuals with developmental disabilities, including administering medications, monitoring vital signs, and performing various nursing procedures.
  • Assist with the admission, transfer, and discharge of individuals, ensuring a smooth and efficient process.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans, ensuring the best possible outcomes for our clients.
  • Participate in mandatory training and in-service training/workshops to enhance therapeutic skills and stay up-to-date on best practices.
  • Perform other duties as required or assigned, which are reasonably within the scope of the duties enumerated above.
Requirements:
  • Requires Illinois license as a practical nurse.
  • Requires one year of practical nursing experience.
  • This class is included as an Upward Mobility Program credential title.
Working Conditions:

The Licensed Practical Nurse II will work in a fast-paced environment, providing care to individuals with developmental disabilities. The successful candidate must be able to physically restrain individuals as necessary to prevent injury to the individual or others. The position requires the ability to work after business hours, weekends, and holidays, as well as utilize office equipment, including personal computers. The Licensed Practical Nurse II must also be able to travel in the performance of job duties and pass the IDHS background check and a drug screen for drugs prohibited from recreational use under Illinois Law.
