***This is a part-time role requiring approximately 24 hours per week, scheduled between Monday and Friday, from 7:00 AM to 3:30 PM. Specific hours will be agreed upon with the selected candidate.

Position Purpose:

The CES Evaluation Specialist is tasked with enrolling individuals and families facing homelessness into the Indianapolis Continuum of Care's Coordinated Entry System (CES). This assessment is crucial for determining eligibility and prioritization for rental assistance and housing support services. Assessments may occur at the Horizon House service center or at partner community organizations. This role collaborates closely with case management and guest services teams, as well as community partners, to connect individuals experiencing homelessness with resources and support aimed at achieving long-term housing stability.

Qualifications:

Education/Training:

  • Bachelor's degree preferred; equivalent experience is acceptable.
  • Fluency in Spanish is advantageous.

Experience/Skills:

  • One to two years of experience in a program serving individuals or families facing homelessness or poverty is preferred (this may include educational, internship, or volunteer experience).
  • Ability to foster professional relationships with clients, agencies, and community partners.
  • Strong written and verbal communication skills are essential.
  • Knowledge of human behavior and the ability to assess client strengths and needs are required.
  • Familiarity with community resources is preferred.
  • Excellent organizational skills with a keen attention to detail are necessary.
  • Proficiency in Microsoft applications and the ability to learn required database tracking systems.

Key Responsibilities:

Assessment and Triage:

  • Conduct comprehensive assessments with a diverse clientele using formats and tools established by the Indianapolis Continuum of Care.
  • Enroll individuals into the CES process and provide timely interventions and referrals to meet immediate needs.
  • Assist in crisis management as assigned.

Care Coordination:

  • Collaborate with co-located partners and community organizations to gather and manage all documentation required for CES assessments.
  • Maintain necessary documentation and client contact to ensure CES assessments remain active, adhering to Continuum of Care guidelines.
  • Participate in weekly Case Conferencing meetings to advocate for clients in the housing referral process.

Record Keeping:

  • Ensure electronic and paper documentation of client records complies with agency and external policies, procedures, and applicable licensing standards.
  • Manage enrollment and exit documentation in accordance with CES guidelines.

Legal Compliance:

  • Adhere to confidentiality protocols and ethical conduct, maintaining appropriate professional boundaries.
  • Take necessary actions according to established organizational protocols and local/state laws.

Education/Training:

  • Attend required agency and community training sessions to secure and maintain the CES Assessor position.
  • Assist in training new staff, community partners, and interns as needed.
  • Provide peer support to colleagues as appropriate.
  • Contribute to agency and team meetings; represent the agency at community meetings when requested.

Work Hours:

  • This is a part-time position requiring 24 hours per week, typically Monday through Friday, with occasional evening or weekend hours as needed.

General Duties:

  • Perform additional tasks as assigned by the Clinical Supervisor, Program Manager, or other agency leadership.

Contacts and Collaborations:

The CES Evaluation Specialist will regularly communicate with Horizon House employees, external vendors, and other stakeholders as necessary to fulfill the responsibilities of the role.
