Job Summary

The Greens at Gastonia is seeking an experienced and skilled RN/LPN MDS Coordinator to join our team. As a key member of our healthcare team, the successful candidate will be responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring timely and accurate completion of Minimum Data Set (MDS) assessments, Care Area Assessments (CAAs), and Interdisciplinary Care Plans.

Key Responsibilities
  • Assessment Coordination: Monitor and guide the completion of assessments in a timely manner, ensuring accuracy and completeness.
  • MDS Management: Involvement and supervision of inputting of MDS assessments, transmission to the state, and obtaining receipt of validation.
  • Care Planning: Schedule and facilitate care plan conferences, and facilitate monthly Quality Management (QM) meetings.
  • Process Management: Manage the Care Planning Process, ensuring seamless coordination and communication among team members.
Requirements
  • Education: Must hold a Nursing Degree from an accredited college or university.
  • Licensure: Current, unencumbered, active RN/LPN license and CPR certification.
  • Experience: At least previous experience in Long Term Care as a MDS Coordinator.
  • Preferred Qualifications: RAC-CT certification.
Skills and Qualities
  • Knowledge: Outstanding knowledge of Case-Mix, Federal Medicare PPS process, and Medicaid reimbursement.
  • Communication: Excellent written and verbal communication skills and critical thinking abilities.
  • Regulatory Knowledge: Solid understanding of current federal, state, and local standards, regulations, and guidelines that govern our facility.
  • Organizational Skills: Ability to organize and prioritize tasks effectively.
