Position Overview:
As a Practical Nurse, you will operate under the guidance of a Registered Nurse, playing a vital role in ensuring the well-being of our clients.

Your responsibilities will encompass routine care, vigilant observation of health conditions, assisting the RN and physicians with treatments, and maintaining open communication with clients and their families.

You will be expected to report any changes in client conditions or additional needs to your supervisor.

Key Responsibilities:
- Delivering personalized 1:1 care in accordance with the client's care plan while upholding their dignity and independence
- Monitoring vital signs and promptly reporting any irregularities to the Case Manager and Physician
- Administering medications, feedings, and other necessary care as outlined in the care plan, along with evaluating the outcomes
- Collaborating with other healthcare professionals such as Physical Therapists, Occupational Therapists, and Speech Therapists to follow established routines
- Upholding infection control protocols and reporting any potential communicable diseases or infections
- Documenting all nursing actions and communications with family members, case managers, and physicians, as well as tracking progress towards established goals
- Coordinating the procurement of supplies, medications, and feedings to ensure availability
- Meeting all patient needs as directed by the physician
- Monitoring incidents involving clients, ensuring timely and accurate completion of incident reports, and informing case managers
- Maintaining medical equipment and reporting any malfunctions for repair
- Performing additional duties as assigned

Qualifications:
- LPN/LPT/LVN Degree from an accredited nursing institution with an active and unrestricted license in the state of practice
- Valid driver's license with a satisfactory driving record and proof of car insurance
- CPR certification
- Experience in medication administration
- Preferred: 1+ years of nursing experience and home health care experience
- Ability to lift and carry items weighing up to 20 pounds
- Capable of bending, stooping, pushing, pulling, sitting, and walking for extended periods
- Strong verbal and written communication skills
