Job Role Overview:

We are seeking a skilled physical therapist to join our Outpatient Wound Care team. The successful candidate will collaborate with fellow therapy professionals in an outpatient environment to deliver comprehensive wound management through various therapeutic approaches aimed at promoting healing, managing swelling, and educating patients on lifestyle modifications to prevent future wounds. Strong communication with referring healthcare providers and multidisciplinary team members is crucial for success in this role. This position offers the chance to enhance skills in a vibrant, specialized setting that encourages continuous learning and professional development.

Key Responsibilities:
  • Assessment:
    • Tailors evaluations to accommodate the patient's age and cultural background.
    • Reviews patient history to inform clinical decisions.
    • Conducts thorough evaluations.
    • Utilizes appropriate standardized and non-standardized assessments.
    • Creates a reasonable and age-appropriate care plan.
    • Establishes client-centered goals that are both realistic and achievable.
  • Intervention:
    • Modifies treatment plans based on the patient's age and cultural context.
    • Delivers timely and suitable interventions based on the patient's physical and emotional needs.
    • Provides education that is centered around the client.
    • Ensures treatments are innovative and effective.
  • Documentation:
    • Maintains accurate and timely records of patient evaluations and treatments in accordance with documentation standards.
    • Secures necessary signatures.
    • Justifies billing, reflecting skilled therapeutic necessity, functional improvements, and progress towards established goals.
  • Productivity:
    • Meets or exceeds established productivity benchmarks.
    • Willingly accepts walk-in patients.
    • Flexibly assists other teams as required.
    • Adjusts scheduling based on patient load.
  • Administrative Duties:
    • Completes ancillary tasks that impact patient care.
    • Ensures timely completion of e-learning and other assigned tasks without reminders.
    • Submits authorization requests and insurance documentation promptly to avoid delays in patient care.
  • Professional Communication and Collaboration:
    • Exhibits effective communication and teamwork with colleagues and staff.
    • Consults with physicians and caregivers regarding patient management as needed.
    • Delegates routine care tasks appropriately.
    • Demonstrates strong verbal and written communication skills with staff, patients, and families.
    • Provides meaningful educational materials to patients and completes necessary interdisciplinary forms.
    • Participates in rounds and care conferences.
    • Responds positively when asked to assist colleagues.

Qualifications:

- Valid Indiana Physical Therapy license or eligibility for licensure

- Preferred experience in wound care within an acute or outpatient healthcare setting

- Current BLS certification
