About the Role

We are seeking a skilled and compassionate Hospital Nurse to join our team at SIU Medicine. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and ensuring a smooth transition from hospital to home or long-term care facility.

Key Responsibilities
  • Provide direct patient care, working under the direction of the CFM Hospital Team to bridge gaps between inpatient and outpatient settings.
  • Establish rapport with patients and caregivers to ensure hospital discharge appointments are made and the importance of follow-up visits is emphasized.
  • Provide patient education and assist with discharge documentation, ensuring a seamless transition of care.
  • Attend in-patient rounds to gain understanding of patient care and treatment, and participate in identifying care needs and linking with applicable services.
  • Identify patients at higher risk for readmission and implement a patient-centered plan of care to promote health improvements.
  • Provide telephone triage for incoming calls from patients, pharmacies, and other healthcare agencies regarding care of discharged patients.
  • Place follow-up calls to patients within 2 days of discharge to confirm status, reconcile medications, and identify any immediate concerns.
  • Conduct additional follow-up calls as needed to support patient goals, reassess progress, and provide further education.
  • Communicate with hospital teams and PCPs to ensure patient follow-up and reconcile medication lists.
  • Maintain accurate and up-to-date documentation for all case management activities.
Qualifications
  • Possession of a valid license as a Licensed Practical Nurse issued by the Illinois Department of Financial and Professional Regulation, Division of Professional Regulation.
  • 6 months of nursing experience in a healthcare facility, such as a hospital, nursing home, medical clinic, or closely related facilities.
