Acute Inpatient Medical Social Worker Specialist

We are seeking a dedicated and compassionate Licensed Master Social Worker (LMSW) to join our team as an Acute Inpatient Medical Social Worker. In this role, you will provide advocacy, education, assessments, and interventions to patients and families in the inpatient hospital setting to achieve an optimal level of care.
Key Responsibilities:- Provide patient-centered care and support to patients and families in the inpatient hospital setting.
- Collaborate with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning to achieve quality clinical outcomes.
- Provide a range of interventions for patients, families, and caregivers, including counseling, crisis intervention, and resource referrals.
- Participate in care coordination for complex cases and collaborate with healthcare team members to optimize resource allocation.
- Licensed Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred.
- Master's Degree in Social Work required.
- Two years of experience in hospital setting or medical social work setting preferred.
- Experience in case management, addiction services, mental health, or population health management desired.
