We are seeking a skilled Social Worker to join our team at Saint Alphonsus Health System. As a Social Worker, you will play a vital role in providing advocacy, education, and interventions to patients and families in our inpatient hospital setting.
Key Responsibilities- Provide assessments and interventions to patients and families to achieve an optimal level of care.
- Coordinate with medical staff and outside organizations to establish specific treatment plans and facilitate transition planning.
- Collaborate with healthcare team members to efficiently allocate resources and provide high-quality care.
- Accurately document assessments, plans, and interventions in the EMR.
- Responsible for coordination of care to mental health and/or substance abuse facilities.
- Responsible for following established patient choice policy.
- Collaborate with physicians and hospital staff through formal and/or informal multidisciplinary case conferences to meet the emotional, mental, physical, and spiritual needs of patients and/or their families/caregivers.
- Licensed Master Social Worker (LMSW) required as defined by their primary work state (Idaho or Oregon), Licensed Clinical Social Worker (LCSW) preferred.
- Master's Degree in Social Work required.
- Two years of experience in hospital setting or medical social work setting preferred.
- Experience in case management, addiction services, mental health, or population health management desired.
