Clinical Case Manager

This position provides a wide array of services to chronically mentally ill adults under the supervision of the CSP Clinical Coordinator. The Clinical Case Manager will provide psychiatric assessment, treatment, and clinical case management services to clients in their homes or communities. The position requires travel throughout Green County and includes a variety of shifts, including late shifts, weekends, and holidays.
Key Responsibilities
- Provide psychotherapeutic, supportive, educative, and psychosocial services to clients
- Develop and coordinate resources to meet clients' basic needs
- Deliver and observe clients' medications as needed
- Document client progress in client records
- Provide representative payee services and consult with outside agencies
- Perform mental status assessments and provide crisis intervention services
- Attend and participate in daily interdisciplinary team meetings
- Develop, implement, evaluate, and revise treatment goals and plans
Requirements
Masters degree in social work, clinical psychology, behavioral science, or psychiatric mental health nursing, or equivalent requirements and experience providing services to the chronically mentally ill population. Possession of a valid driver's license and access to personal transportation is required.
