Job Title: Certified Hospice Aide

Certified Hospice Aide is a paraprofessional role that plays a vital part in the interdisciplinary team, working under the supervision of a registered nurse to provide care and support to patients and their families.

Job Summary:

The Certified Hospice Aide is responsible for observing patients, reporting observations, and documenting care provided. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a team environment.

Key Responsibilities:
  • Observe patients and report observations to the interdisciplinary team
  • Provide personal care and support to patients as needed
  • Document care provided and observations made
  • Work collaboratively with the interdisciplinary team to develop and implement patient care plans
Requirements and Qualifications:

To be successful in this role, you will need:

  • An active hospice aide certification with the state
  • At least 1-3 years of experience in hospice, palliative care, or end-of-life care
  • Knowledge of hospice care and services provided to patients and families
  • Proficiency in MS Office applications and ability to learn department-specific software systems
Core Competencies:
  • Communication: ability to effectively communicate with patients, families, and healthcare team members
  • Customer Service: ability to provide excellent customer service to patients and families
  • Emotional Intelligence: ability to manage emotions and interact effectively with others
  • Time Management: ability to manage time effectively and prioritize tasks
