Job Title: Registered Nurse - Case Management

VNA Health Group is seeking a skilled Registered Nurse to join our team as a Case Manager. As a Case Manager, you will play a vital role in providing assessment and referrals for individuals living with HIV to increase access to medical care and other needed resources.

Responsibilities:
  • Supports the organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationships with co-workers.
  • Performs assessments of individual needs and develops and documents appropriate plans of care.
  • Anticipates insurance lapses and assists with required insurance and medication coverage applications.
  • Assesses for related program eligibility, mental health, housing services, substance use services, and other entitlements.
  • Documents all required data into the reporting systems, including CareWare, and provides reports as requested by the Program Manager.
  • Provides ongoing follow-up and evaluation of the effectiveness of services being provided.
  • Adheres to the procedures and documentation as required by Part B Standards of Care for Case Management.
  • Adheres to agency infection control and safety policies.
  • Adheres to the organization's policy in regards to absenteeism and appearance.
Qualifications:
  • Bachelor's degree in Social Sciences or related field.
  • 1+ years of HIV/AIDS work-related experience.
  • Knowledge of cultural and lifestyle diversity issues.
  • Strong organizational and interpersonal skills.
  • Bi-Lingual Spanish preferred.
