About the Role:
  • Provide assessment and referrals for individuals living with HIV to increase access to medical care and other needed resources.
  • Support the organization's mission by striving for excellence in all aspects of the job with a focus on positive interpersonal relationships with co-workers.
  • Perform assessments of individual needs and develop and document appropriate plans of care.
  • Anticipate insurance lapses and assist with required insurance and medication coverage applications.
  • Assess for related program eligibility, mental health, housing services, substance use services, and other entitlements.
  • Document all required data into the reporting systems, including CareWare, and provide reports as requested by the Program Manager.
  • Provide ongoing follow-up and evaluation of the effectiveness of services being provided.
  • Adhere to the procedures and documentation as required by Part B Standards of Care for Case Management.
  • Adhere to agency infection control and safety policies.
  • Adhere to the organization's policy in regards to absenteeism and appearance.
Requirements:
  • Bachelor's degree in Social Sciences or a related field.
  • 1+ years of HIV/AIDS work-related experience.
  • Knowledge of cultural and lifestyle diversity issues.
  • Strong organizational and interpersonal skills.
  • Bi-Lingual Spanish preferred.
