- Provide Clinical Care: Under the direction of physicians, you will participate in patient care by performing clinical duties, responding to patient inquiries, and coordinating care in the medical home and across the continuum of care.
- Coordinate Care: You will work closely with patients, physicians, and other healthcare professionals to ensure seamless care coordination and excellent patient outcomes.
- Communicate Effectively: You will possess excellent communication and interpersonal skills, enabling you to interact effectively with patients, physicians, supervisors, and other staff members.
- Manage Time and Priorities: You will demonstrate skill in time management, problem-solving, multitasking, prioritizing, and medical care coordination.
