We are seeking a highly skilled and compassionate Care Coordinator to join our team at Spurwink Services. As a Care Coordinator, you will play a vital role in integrating, coordinating, and monitoring the active treatment programs for our clients with intellectual disabilities.
Key Responsibilities:
- Ensure the active treatment is consistently implemented for all residents.
- Act as a liaison between outside entities, including family, guardians, community support programs, and others, to coordinate treatment-related objectives.
- Monitor progress with individual program plans and initiate changes as necessary.
- Provide recommendations to team members related to treatment goals and objectives.
- Ensure adequate environmental supports and assistive devices are present to promote independence.
Requirements:
- Bachelor's Degree in a Human Services field.
- At least one year of experience working directly with persons with intellectual disabilities or other developmental disabilities.
