Our specialty hospitals provide the highest level of care, similar to that of traditional hospitals or intensive care units, but designed for extended recovery periods. Our healthcare professionals play a crucial role in the recovery journey for patients who are chronically ill, critically injured, or medically complex.


Position Overview:

The Wound Care Coordinator oversees the facility's wound care program, ensuring adherence to standards of care and practice for wound, ostomy, and continence patient needs.

This role includes providing direct patient care, assisting nursing staff, and serving as a consultative resource for affiliated healthcare organizations.


Key Responsibilities:

  • Develop and implement the facility's wound care program in alignment with national standards, including patient care protocols, documentation tools, and referral criteria.
  • Establish care standards, competencies, policies, and procedures to ensure quality, cost-effective, and efficient wound care for all clinical staff.
  • Serve as the primary consultant to Wound Care Clinicians, offering training and support to enable clinical staff to assess wounds, recommend protocols, and initiate care plans effectively.
  • Evaluate all wounds upon admission and continuously to formulate treatment plans and identify potential issues early.
  • Provide consultation and assessment for patients with pressure ulcers and any wounds that do not show measurable healing signs within two weeks.
  • Collaborate with physicians to assist primary nurses in developing comprehensive wound management plans.
  • Recommend changes to wound care orders to physicians, providing evidence-based research support as necessary.
  • Review patient charts to ensure accurate documentation of wound assessments, care plans, and treatments, and investigate adverse events related to wounds.
  • Participate in clinical outcome monitoring and performance improvement initiatives.
  • Work with Support Center Clinical Operations staff to maintain a cost-effective wound care formulary.
  • Assess products for cost-effectiveness in the hospital setting and make recommendations.
  • Collaborate with all relevant disciplines to develop discharge plans.
  • Review culture results and confer with physicians on care plans.
  • Educate staff on available products for hospital use and provide education to families on patient care.
  • Engage in business development activities to promote Kindred Healthcare facilities as the preferred provider for wound care.
  • Involved in marketing the wound care program through outcomes and physician relationships.

Qualifications:

  • Degree from an accredited nursing program leading to licensure as a Registered Nurse.

Licenses/Certifications:

  • Current state RN license.
  • Professional certification in Wound, Ostomy, and Continence Nursing (WOCN) or Certified Wound Specialist (CWS), or obtain certification within 12 months of employment. BLS certification is required.

Experience:

  • A minimum of five (5) years of licensed professional nursing experience, including prior experience in acute care settings.
