Job SummaryThe Counselor Assistant/Case Manager will primarily be responsible for the case plan of patients who are admitted to Adult Outpatient (ADOP). This role will work closely with the lead counselor for chemical dependency counseling, case management, and treatment planning for patients.
Responsibilities- Develop and implement individualized case plans for patients in the Adult Outpatient program
- Conduct chemical dependency counseling and case management services for patients
- Collaborate with the lead counselor to ensure comprehensive treatment planning and coordination of care
- Provide education and support to patients and their families on chemical dependency and recovery
- Document patient progress and maintain accurate records
- High School Diploma or G.E.D.
- First-hand experience with a twelve-step recovery program or comparable experience required
- Associate or bachelor's degree in human services or behavioral science with some paid volunteer clinical experience or internship in the field of chemical dependency treatment, or equivalent preferred
- Valid State of Ohio Chemical Dependency Counselor Assistant (CDCA) certification required
- Salary commensurate with experience
