Job Summary

The Primary Care Licensed Practical Nurse (LPN) plays a vital role in delivering high-quality nursing care to patients in a community setting. Working closely with healthcare providers, the LPN will provide direct patient care support, coordinate annual wellness visits, and perform medication reconciliation. This role requires strong communication and interpersonal skills, as well as the ability to work independently and collaboratively as part of a team.

Key Responsibilities
  • Provide direct patient care support in a variety of settings, utilizing primary and palliative care principles, trauma-informed care, and promoting patient well-being and empowerment.
  • Coordinate, schedule, and conduct annual wellness visits with patients, families, and caregivers.
  • Perform medication reconciliation through patient interview, review of medical records, and consulting with other healthcare providers involved in the patient's care.
  • Collaborate with providers to resolve discrepancies or address any care gaps and recommended preventative measures such as mammograms, colonoscopies, lab tests, etc.
  • Engage patients and families in advance care planning, including discussions about goals of care and answering general questions about advanced directives and assisting patients with documentation if requested.
  • Educate patients about their health conditions, treatment options, and self-management strategies to promote active participation in their care.
  • Coordinate, schedule, and accurately perform point-of-care testing; communicate findings to the healthcare team for further evaluation and treatment.
  • Communicate test results with patients when deemed appropriate by the provider or payer.
  • Assess vaccination needs, discuss vaccination options, obtain informed consent, prepare and administer vaccines, monitor for adverse reactions, and provide post-vaccine education and documentation.
  • Recognize signs and symptoms of common and routine medical conditions/illnesses, abnormal versus new/existing, and collaborate with the patient's provider when identified.
  • Work closely with members of the primary care interdisciplinary team or payor staff to ensure coordinated and comprehensive care and optimal scheduling of patient visits.
  • Collaborate with the healthcare team to develop a responsive plan of action to address crises effectively and ensure patient safety.
  • Practice within the scope of license, individual competencies, certification, or registration, where applicable, and as required by law and regulation.
  • Provide timely and effective patient documentation, including data collection.
  • Identify and address organizational, cultural, and other barriers to best practice strategies.
  • Assist in mentoring newly hired staff; may provide coaching and training.
  • Provide backup coverage for primary care nurses, within LPN scope of practice.

Requirements
  • Graduate of an accredited LPN training program.
  • Current unencumbered LPN license in the State of Oregon.
  • Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle.
  • CPR certification at hire or within 6 months in position.
  • Minimum 2 years' experience in primary care, geriatrics, home health, or hospice.
Preferred Qualifications
  • Knowledge of Federal, State, and Local regulations, and HIPAA privacy rules and regulations.
  • General computer skills and a working knowledge of Electronic Medical Records systems, MS Office, Outlook, and the internet.
  • Excellent interpersonal skills.
  • Ability to work both independently and collaboratively.

  • Ability to accept direction and feedback, as well as tolerate and manage stress.
  • Ability to see, read, hear, and speak clearly for at least 6 hours/day.
  • Ability to stand, walk, sit, push, pull, and perform repetitive finger and wrist movement for at least 3-6 hours/day.
  • Ability to lift, carry, crouch, crawl, reach, climb stairs, and/or pinch small objects for at least 3 hours/day.
