Infusion Intake Nurse

We are a healthcare system that believes in the importance of partnership between wellness and sickness. Our team is dedicated to providing expert guidance to patients as they transition from hospital care to home infusion therapy.
Job Summary:This role offers a dynamic environment where you can collaborate with team members to provide care to patients transitioning from the hospital to their homes. As an Infusion Intake Registered Nurse, you will be an integral part of the referral process, utilizing your clinical expertise, great communication skills, and attention to detail to ensure our patients are set up for success.
Responsibilities:- Successfully fulfill the criteria and experience profile for Registered Nurse, in addition to the responsibilities outlined in this job description.
- Participate in discharge planning of patients and maintain proficiency through process improvement and operational efficiency.
- Collaborate with departmental and interprofessional teams to ensure smooth transition of care.
- Associate's Degree (Required)
- RN - Registered Nurse - Ohio Board of Nursing
- 2-3 years of bedside/floor nursing experience in IV Therapy (Preferred)
- Transitions of Care experience is a plus (Preferred)
This position will be located on-site at the home infusion pharmacy. We offer a supportive team environment and a great learning culture with tenured staff of 20+ years to support onboarding needs.
