Job Description Summary:

Schedule: 2-3 shifts a week Mon-Fri, no weekends & no holidays
• Hours of Operation: 9:00am - 5:30pm

Come join the OhioHealth Infusion Pharmacy team This role offers a dynamic environment where you can collaborate with team members to provide care to patients transitioning from the hospital to their homes. You will be an integral part of the referral process to ensure our patients are set up for success by utilizing your clinical expertise, great communication skills and attention to detail. In addition, you will have the opportunity to provide patient education on how to setup their IV hookups independently.

You will be joining a collaborative team of Pharmacists, Nurses, and Technicians. Orientation is typically 12 weeks and we have a great learning culture with tenured staff of 20+ years to support onboarding needs.

Responsibilities Overview:

-Successfully fulfills the criteria and experience profile for Registered Nurse, in addition to the responsibilities outlined in this job description. The Pharmacy Intake RN demonstrates technical, time management, communication, coordination, clinical excellence, and leadership skills necessary to support business needs regarding Infusion Pharmacy referrals. This position ensures physician orders are in place, performs ICD-10 coding, processes referral information, reviews drug qualifications, and participates in discharge planning of patients. Maintains proficiency through process improvement and operational efficiency. Participates in mutual communication with departmental and interprofessional teams to ensure smooth transition of care. This position will be located on-site at the home infusion pharmacy.

Minimum Qualifications:

Associate's Degree (Required)RN - Registered Nurse - Ohio Board of NursingOhio Board of NursingOhio Board of Nursing

Preferred Experience:
  • 2-3 years of bedside/floor nursing experience in IV Therapy
  • Transitions of Care experience is a plus
