Job Summary

We are looking for a highly motivated and experienced Rehabilitation Program Manager to join our team. The successful candidate will be responsible for managing and administering all aspects of rehabilitation services for our communities, as well as maintaining a clinical caseload.

Key Responsibilities
  • Manage and administer rehabilitation services for our communities, including developing and implementing clinical programs and services.
  • Maintain a clinical caseload and provide direct patient care as needed.
  • Develop and maintain relationships with community staff, home health staff, hospice staff, physicians, nurses, and other healthcare professionals.
  • Ensure compliance with federal, state, and local laws, rules, and regulations related to therapy service delivery in Assisted Living, Independent Living, and CCRC settings.
  • Develop and implement quality improvement programs to ensure high-quality patient care and services.
  • Participate in patient care meetings to facilitate open communication and ensure effective care coordination.
  • Provide education and training to community staff, residents, and their families on therapy services and facility programs.
  • Develop and implement marketing strategies to promote therapy services and programs.
  • Supervise and manage rehabilitation staff, including Rehabilitation Technicians, students, and volunteers.
  • Conduct performance appraisals and provide feedback to staff to ensure high-quality patient care and services.
Requirements
  • Bachelor of Arts or Science Degree or a Master's Degree in Occupational Therapy and Physical Therapy.
  • Valid state professional licensure.
  • Assumes responsibility for ongoing continuing education and professional development.
