The Physician Assistant (PA) or Nurse Practitioner (NP) will be responsible for delivering high-quality healthcare services to patients across various settings, including telehealth, in-person, and remote monitoring environments. The PA will work closely with other healthcare team members to diagnose, treat, and manage acute and chronic conditions.

Key Responsibilities:

  • Patient Assessment: Conduct comprehensive assessments, including physical exams, review of medical histories, and analysis of diagnostic tests.
  • Diagnosis and Treatment: Diagnose and manage acute and chronic illnesses, injuries, and infections. Develop personalized treatment plans and prescribe medications as necessary.
  • Preventive Care: Provide preventive care services such as vaccinations, screenings, and counseling on lifestyle and health maintenance.
  • Patient Education: Educate patients and their families on their conditions, treatment options, medications, and self-care strategies.
  • Care Coordination: Collaborate with physicians, Nurse Practitioners, specialists, and Care Navigators to provide coordinated care and manage patient treatment plans.
  • Telehealth Services: Conduct virtual consultations, handle follow-up appointments, and coordinate referrals via the telehealth platform.
  • Prescription Management: Manage and monitor patient medications, adjusting dosages and renewing prescriptions when necessary.
  • Record Keeping: Maintain accurate, timely, and compliant patient records, adhering to healthcare regulations, including HIPAA.
  • Community Health Initiatives: Support CHI programs to address healthcare disparities in underserved communities by delivering essential services and health education.
  • Compliance: Ensure all patient care and documentation comply with federal, state, and healthcare regulations.

Requirements:

  • Education: Graduate from an accredited Physician Assistant or Nurse Practitioner program.
  • Licensure: Valid Physician Assistant or Nurse Practitioner license in New York.
  • Experience: 2-5 years of clinical experience as a Physician Assistant or Nurse Practitioner, preferably in telehealth or primary care.
  • Technology Skills: Proficiency in EMR systems (AdvancedMD) and telehealth platforms.
  • Regulatory Knowledge: Strong understanding of HIPAA, state healthcare regulations, and telehealth standards.
  • Bilingual (Preferred): Fluency in Spanish is preferred for effective communication with diverse patient populations.
