Advanced Practice Pharmacist

At Sutter Health, we are seeking an experienced Advanced Practice Pharmacist to join our team. As a key member of our healthcare team, you will play a vital role in providing high-quality patient care and contributing to the development of our pharmacy services.
Responsibilities:- Initiate, modify, and discontinue medications within a Collaborative Practice Agreement as defined by the California State Board of Pharmacy.
- Focus on the management of complex disease states to help patients reach optimal therapeutic goals.
- Simplify patient medication regimens, minimize the risk of harmful medication interactions, and educate patients to enhance adherence and clinical outcomes.
- Develop and implement policies, procedures, and guidelines to ensure the delivery of high-quality patient care.
- Collaborate with medical providers to ensure seamless patient care and communication.
- Perform patient care assessments, order and interpret laboratory tests, prescribe, modify, and discontinue medication therapies.
- Monitor clinical outcomes, communicate with medical providers, and refer patients to other healthcare providers as needed.
- Graduate of an accredited school of pharmacy in the U.S.
- 12 years of recent relevant experience.
- PHARMR-Registered Pharmacist upon hire.
- ASHP-Post Graduate Year 1 ASHP General Pharmacy Residency upon hire.
