Advanced Practice Pharmacist

We are seeking a highly skilled and motivated Advanced Practice Pharmacist with Oncology experience to join our team at Soliant in California.
About the Role:This direct hire position offers a sign on bonus and requires an active California license and the required experience to be considered for this role.
Key Responsibilities:- Initiate, modify, and discontinue medications within a Collaborative Practice Agreement.
- Manage complex disease states to help patients achieve optimal therapeutic goals.
- Simplify patient medication regimens, minimize the risk of harmful interactions, and educate patients to enhance adherence and clinical outcomes.
- Develop policies, improve continuous processes, provide continuing education, and contribute to clinical and medical staff development.
- Serve as a preceptor for Pharmacy APPE students and residents.
- Perform patient care assessments, order and interpret laboratory tests, and prescribe, modify, and discontinue medication therapies.
- Monitor clinical outcomes, communicate with medical providers, perform electronic documentation, and refer patients to other healthcare providers.
- Doctor of Pharmacy (PharmD) degree.
- Completion of PGYI + PGYII or 4+ years of current ambulatory care oncology experience.
- Board Certified Oncology Pharmacist (BCOP) preferred.
- Active/clear California pharmacist license.
