Job Summary

The Registered Dietitian serves as a vital member of the PACE Interdisciplinary Team at Element Care. This role works under the direction of the Nutrition Coordinator and the Site Coordinator, providing comprehensive nutrition education and assessments to PACE participants. As a key contributor to the team, the Registered Dietitian plays a crucial role in planning, implementing, and evaluating care plans that meet the objectives, standards, and policies of the PACE model of care.

Essential Functions
  • Conduct thorough assessments of participants' nutritional status at enrollment, annually, and as needed, to ensure optimal care.
  • Share relevant participant information with the Interdisciplinary Team to facilitate informed decision-making.
  • Collaborate with the team to plan dietary supplements for high-risk participants, ensuring their nutritional needs are met.
  • Report observations and participant changes, including behavioral changes, to ADH staff clinicians in a timely manner.
  • Participate in family meetings and case conferences as needed, providing expert nutrition guidance.
  • Provide follow-up treatment and teaching as necessary, ensuring participants receive comprehensive care.
  • Makes home visits as requested, demonstrating a commitment to participant-centered care.
  • Interact with participants, caregivers, and families in a professional and courteous manner, fostering strong relationships.
  • Interact with other providers and community professionals in a courteous and professional manner, promoting collaboration and communication.
  • Maintain accurate and timely records and documentation, adhering to Element Care's standards.
  • Ability to pass a fit test, with a mask seal being critical; incumbent must not have facial hair that interferes with the respirator seal.
Additional Responsibilities
  • Participate in Quality Improvement projects and assessments as needed, contributing to the continuous improvement of care.
  • Participate in Element Care Committees as required, communicating relevant information as necessary.
  • Complete required annual Health Stream trainings in a timely manner, staying up-to-date on best practices.
  • Adhere to all policies and procedures, supporting Element Care's Primary Vision Statement.
  • Adhere to the confidentiality policy, including HIPAA guidelines, to protect participant information.
  • Be flexible to perform other related jobs as reasonably requested.
Essential Job Requirements
  • Master of Science Degree in Nutrition (preferred)
  • Registered Dietitian through CDR and current state licensure in the Commonwealth of Massachusetts.
Experience
  • Two years of experience working in a community setting.
  • One year of experience working with the geriatric population.

  • Ability to lead independently and work well with others, fostering a collaborative team environment.

  • Ability to quickly recognize situations/problems before they escalate, demonstrating proactive problem-solving skills.
