Vanna Health is seeking a skilled Medical Social Worker to join our interdisciplinary team. This dynamic individual will play a vital role in supporting individuals living with serious mental illness throughout their recovery journey.
The successful candidate will be responsible for coordinating various aspects of the recovery process, including discharge planning, care coordination, and assessing member eligibility for benefit programs.
This role will involve working closely with our community team, including coaches, nurses, housing specialists, and psychiatrists, to ensure a comprehensive approach to member care.
Key Responsibilities:
- Discharge Planning: Lead discharge planning for Vanna community members at ERs, CRCs, and hospitals.
- Care Coordination: Coordinate referrals and support appointment scheduling with local primary care, behavioral health, and other provider types for members.
- Supportive Counseling: Provide supportive counseling to members when appropriate.
- Assessments: Conduct detailed assessments of members, including suicide assessments.
- Member Support: Provide appointment reminders and ensure members have transportation to attend appointments.
- Needs Assessment: Assess member needs and support systems, including social determinants of health.
- Benefits Support: Support member applications for SSI, SSDI, SSA, welfare, housing, Medicaid, and other related programs.
- Transportation: Assist members in accessing transportation by connecting them to transportation benefits and providing transportation with Vanna vehicles when required.
- Documentation: Adhere to documentation protocols for discharge planning, application enrollment, care coordination, and other documentation requirements.
- Team Collaboration: Collaborate closely with the community team to support the coordination of member care.
- Process Improvement: Provide feedback to Vanna technology and operations teams on existing processes and suggest improvements over time.
About You
We are looking for a passionate and experienced Medical Social Worker who is committed to supporting individuals living with serious mental illness. The ideal candidate will have:
- 3+ years of experience in Case Management or Care Coordination.
- Experience working in mental health and/or primary care in and around the Philadelphia, PA area.
- A strong belief in the recovery potential of individuals with serious mental illness.
- Excellent organizational skills to prioritize tasks and maintain a schedule.
- Experience working in a multidisciplinary team setting and leading change efforts.
- A comfort level working in ambiguity with a willingness to learn and adapt to new structures.
- Basic computer and phone skills, including proficiency in Google, Slack, and Vanna EHR tools.
- A valid driver's license and comfort transporting members in company vehicles.
Education Requirements:
- Licensed Clinical Social Worker in the state of Pennsylvania.
Nice to Have:
- Bilingual in Spanish and English to ensure we meet members in their most comfortable language.
