Career OpportunitiesWe are currently seeking a full-time Medical Case Manager at one of our largest Health Centers. This role requires strong communication skills and the ability to work effectively with patients from diverse backgrounds.
Job DescriptionThe successful candidate will provide comprehensive counseling on various health-related issues, including basic health maintenance, STD/HIV education, prenatal care, options counseling, and terminal illness coping/planning. They will also collaborate with other healthcare professionals to ensure seamless patient care.
RequirementsTo be considered for this position, you must possess a Bachelor of Social Work or a related field. Alternatively, applicants with a Bachelor's degree in another discipline may apply if they have at least two years of experience in case management. We require a minimum of one year of clinical experience in an ambulatory healthcare setting or case management, as well as two years' experience in chart review.
Excellent communication skills are essential for success in this role. Local candidates are preferred, but we welcome applications from individuals who share our commitment to patient-centered care.
