Nurse Practitioner

Job Summary: We are seeking a highly skilled Registered Nurse to join our multidisciplinary team as a PACT (Program of Assertive Community Treatment) registered nurse. The successful candidate will be responsible for conducting psychiatric assessments, assessing physical health needs, making appropriate referrals to community physicians, and providing management and administration of medication in conjunction with the nurse practitioner/psychiatrist. Additionally, the candidate will provide a range of treatment, rehabilitation, and support services, and share shift-management responsibility with other staff. Embracing and ensuring Washington State PACT fidelity standards are met is a critical attribute required of this position.
Key Responsibilities:
- Conduct psychiatric assessments and develop treatment plans in collaboration with the multidisciplinary team.
- Assess physical health needs and make appropriate referrals to community physicians.
- Provide management and administration of medication in conjunction with the nurse practitioner/psychiatrist.
- Provide a range of treatment, rehabilitation, and support services to patients and their families.
- Share shift-management responsibility with other staff and ensure continuity of care.
Requirements:
- Registered Nurse with associate, bachelors, or master's degree in nursing required.
- One year experience in mental health evaluation, treatment, and rehabilitation preferred.
- Experience with co-occurring disorders and/or home health preferred.
- Licensed as a RN in Washington State required.
- Current CPR required.
- Must have understanding of the characteristics and problems of adults with severe mental illnesses and knowledge of the PACT model.
- Must have a valid Washington State Driver License with personal transportation and auto insurance.
- Evidence of support and fundamental belief in National Consensus Statement on Mental Health Recovery.
