Job Responsibilities

As a Licensed Practical Nurse with IntelyCare, you will be responsible for:

  • Providing high-quality patient care in post-acute facilities
  • Tracking patient wellbeing and monitoring vital signs
  • Conducting intakes and documenting health concerns and medical history
  • Administering and verifying medications, and monitoring their effects
  • Reviewing patient charts and medical records
  • Assisting with the cultivation of a safe and comfortable facility environment
Qualifications and Skills

To be successful as a Licensed Practical Nurse with IntelyCare, you will need:

  • A current Connecticut state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school graduation, GED, or equivalent
  • 1 year of experience as an LPN or in a similar role
