One Community Health is seeking a highly skilled Pediatric Nurse Practitioner to join our team in an outpatient setting. As a key member of our healthcare team, you will provide comprehensive medical management to patients ranging from newborns to 21 years old.
Key Responsibilities:- Deliver direct pediatric medical management to patients in an outpatient setting, including comprehensive medical consultations, examinations, diagnoses, and management of acute and chronic conditions.
- Maintain routine well visits, vaccinations, and preventative care for patients according to organization guidelines.
- Provide effective updates through communication and education of referring providers, patients, and family members on medical management plans, clinical progress, and prognosis.
- Participate in professional development through education and mentorship of ancillary staff and other allied professionals.
- Coordinate and/or participate in integrated specialty and subspecialty consultations and interdisciplinary care conferences as needed.
- Master's Degree in Nursing
- CA NP Licensure in good standing
- Current DEA number
- Two-plus years related experience preferred
- Pediatric Nurse Practitioner (PNP) certification - Primary Care preferred
- Experience working with diverse patient populations and familiarity with trauma-informed systems and restorative practices
