ACCS Intensive Case Manager Professional

Eliot Community Human Services is a leader in the Behavioral Health realm, providing an integrated and comprehensive network of services to adults with psychiatric disabilities.
We are seeking an Intensive Case Manager to join our multi-disciplinary team to provide clinical interventions to individuals with severe and persistent mental illness living independently.
The Intensive Case Manager will engage each individual in increasing motivation to achieve their goals, empowering them to maintain community stability and improve their quality of life.
Key Responsibilities:
- Provide community outreach to each individual as part of the multi-disciplinary team
- Provide side-by-side interventions that focus on problem-solving, skill training, modeling, optimism, and encouragement
- Work with the team and each individual in the development of a person-centered treatment plan
- Assist each individual in being fully integrated in their communities by supporting them in the development of natural supports
- Empower each individual to exercise control over their lives, focused on their treatment, leisure activities, employment, and personal recovery
- Provide on-call coverage on evenings and weekends as scheduled
Requirements:
- Bachelor's degree in Psychology or Social Work or a related field
- Valid driver's license
- Reliable vehicle
- Ideally 2-3 years prior experience working with adults having severe and persistent mental illness
