Nurse Case Manager

**Job Summary:**
We are looking for a dedicated and compassionate Registered Nurse to join our Case Management team. As a Case Manager, you will work closely with patients, families, and healthcare teams to provide high-quality care and coordinate services.
**Responsibilities:**
* Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans
* Conduct thorough assessments and evaluations to identify patient needs and develop strategies for care coordination
* Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
* Utilize strong problem-solving skills to address complex patient needs and develop creative solutions
* Work collaboratively with interdisciplinary teams to ensure high-quality patient care
**Requirements:**
* RN licensure in the state of Massachusetts
* Minimum of 2 years Med/Surg experience
* Case Manager certification preferred
* ADN required, BSN strongly preferred
* Acute hospital experience preferred
