Job Summary

We are seeking a highly organized and detail-oriented Medical Records Clerk to join our team at Reeves County Hospital. As a Medical Records Clerk, you will be responsible for processing birth certificates, managing medical records, and providing exceptional customer service to our patients and healthcare providers.

Key Responsibilities
  • Birth Certificate Processing
    • Interview parents to obtain accurate and complete information for birth certificates
    • Complete electronic birth certificates using state-provided software, acquire signatures from parents, and transmit to the Bureau of Vital Statistics
    • Complete Acknowledgement of Paternity documentation when required, per state regulations
  • Release of Information
    • Assist walk-in patients with completion of written authorization for release of information
    • Monitor the HIM fax machine for incoming requests from patients or medical facilities
    • Ensure authorizations are valid, according to hospital policy and regulatory guidelines
    • Process, document, and scan requests, authorizations, or other legal documentation within the electronic medical record system
  • Scanning
    • Prepare patient medical records for scanning by ensuring they are properly identified (name, DOB, MR#, acct #)
    • Scan signed documents to the appropriate document type within the electronic medical record
  • Telephone
    • Answer calls promptly and courteously and assist the caller if able
    • Provide written messages to appropriate staff as needed
    • Direct all calls appropriately with minimal disruption
  • Projects
    • Assist with other projects as assigned, including but not limited to purging and packing archived records, etc.
Requirements
  • High School graduate or equivalent
  • Typing speed of 45 wpm
  • Familiar with operating personal computers and knowledge of general clerical duties
  • One year experience in an HIM department preferred
  • Knowledge of medical terminology preferred
