We are seeking a highly skilled Registered Nurse to join our Care Management team at River City Center in Decatur, AL. As a Care Management RN, you will play a vital role in providing exceptional patient care and collaborating with our interdisciplinary team to develop effective care plans.

Key Responsibilities
  • Nursing Assessments and Care Planning: Perform comprehensive nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to develop individualized care plans that address the unique needs of each patient.
  • Goal Setting and Intervention: Establish realistic, measurable short- and long-term patient goals and implement interventions to minimize rehospitalizations and optimize patient outcomes.
  • Leadership and Supervision: Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
  • Communication and Collaboration: Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
Qualifications
  • Education and Licensure: Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
  • Certifications: CPR Certification is required.
