Key Responsibilities- Nursing Assessments and Care Planning: Perform comprehensive nursing assessments and collaborate with the nursing team, other disciplines, patients, and families to develop individualized care plans that address the unique needs of each patient.
- Goal Setting and Intervention: Establish realistic, measurable short- and long-term patient goals and implement interventions to minimize rehospitalizations and optimize patient outcomes.
- Leadership and Supervision: Delegate patient care responsibilities to staff and supervise them to ensure that tasks and interventions are implemented effectively.
- Communication and Collaboration: Communicate patient information with assigned staff and between shifts to ensure seamless care delivery.
- Education and Licensure: Must be a graduate of an approved school of nursing and currently licensed by the State Board of Nursing.
- Certifications: CPR Certification is required.
- Competitive Compensation: Pay Target: $36 - $40 per hour.
