MDS Coordinator Job Description

The MDS Coordinator plays a vital role in ensuring the accurate and timely collection of data for the resident assessment instrument, PPS, and CAAs. This position requires strong leadership skills, effective communication, and a deep understanding of the MDS process.

Key Responsibilities:
  • Coordinate data collection for the resident assessment instrument, PPS, and CAAs
  • Ensure compliance with current professional practice standards, physician's orders, and local, state, and federal regulations
  • Provide education and training to staff on minimum data set requirements
  • Lead the interdisciplinary team in ensuring resources affecting RUG levels are provided and communicate opportunities for improvement
  • Track, schedule, and ensure completion of MDS assessments and other components of the RAI process
  • Direct the interdisciplinary team in the development, revision, and facilitation of resident-specific care plans
  • Implement and evaluate the organization's restorative nursing programs
  • Monitor and audit MDS', CAAs, and care plans completed by others to ensure accurate and appropriate data is collected
Requirements:
  • Graduate of an accredited school of nursing and successfully passing registered nursing or practical nursing licensure exam
  • At least twelve months of experience or equivalent combination of education and experience
  • Current Registered Nurse or Licensed Practical Nurse license in West Virginia and Ohio required
Work Environment:

The MDS Coordinator will work in a fast-paced environment, interacting with residents, families, employees, vendors, and public. The noise level can range from quiet to loud, and the employee may be exposed to airborne viruses and bacteria, as well as blood-borne, fecal-borne, and other bodily pathogens carried by residents.
