MDS Coordinator for Orchards of Alliance

The Orchards is seeking a highly skilled MDS Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for coordinating data for input of the resident assessment instrument, PPS and CAAs and the care plan process within the scope of nursing practice.
Key Responsibilities:- Coordinate data for input of the resident assessment instrument, PPS and CAAs and the care plan process within the scope of nursing practice.
- Ensure compliance in accordance with current professional practice standards, physician's orders, The Orchards' policies and procedures and local, state and federal regulations and requirements.
- Train and educate staff on minimum data set requirements.
- Direct the Interdisciplinary Team in ensuring resources affecting RUG levels are provided and communicates opportunities for improvement.
- Track, schedule, and ensure completion of MDS assessments and other components of the RAI process, in accordance with OBRA and Medicare schedules.
- Direct and facilitate communication among the Interdisciplinary Team in relation to scheduling changes, established MDS reference dates and deadlines for completion.
- Complete or oversee completion of admission and discharge tracking forms, Medicare certifications and maintains a system for identification of specific dates in relation to admissions, re-entry, and discharge.
- Implement and evaluate the organization's restorative nursing programs.
- Monitor and audit MDS', CAAs, and care plans completed by others to ensure accurate and appropriate data is collected.
- Implement and Evaluate of Quality Measure management activities for the organization's skilled nursing facilities.
- Graduate of an accredited school of nursing and successfully passing registered nursing or practical nursing licensure exam.
- At least twelve months of experience or equivalent combination of education and experience.
- Current Registered Nurse or Licensed Practical Nurse license in West Virginia and Ohio required.
