We are seeking a compassionate and dedicated Clinical Social Worker to join our dialysis team at Health Systems Management, Inc.

Responsibilities
  • Provide direct and indirect interventions to pre-dialysis and chronic dialysis patients.
  • Collaborate with the multidisciplinary healthcare team to assist patients in reaching their fullest rehabilitative potential.
  • Communicate with patients and their support system to establish a plan of care.
  • Conduct comprehensive psychosocial assessments.
  • Assess family dynamics and identify the need for further interventions.
  • Utilize community resources to meet patient/family concrete needs.
Requirements
  • Master's Degree in Social Work required.
  • Licensed in Georgia.
  • Ability to solve practical problems and deal with a number of concrete variables in situations.
  • Ability to work independently and plan/organize priorities autonomously.
  • Willingness to work a flexible schedule and to fill in when needed.
  • Excellent bedside manner and communication skills.
