About the Role

This position involves working within a group home setting, where you will be responsible for delivering high-quality occupational therapy services to individuals who require support with daily living activities. Your primary focus will be on creating personalized treatment plans that cater to each person's unique needs, goals, and abilities.

The successful candidate will possess excellent communication and interpersonal skills, enabling them to collaborate effectively with multidisciplinary teams, including direct care staff, families, and healthcare professionals. Additionally, they will have a strong ability to adapt to changing situations and priorities, while maintaining a patient-centered approach.

Key Responsibilities
  • Review medical records and verify orders from medical directors to inform occupational therapy interventions;
  • Communicate with supervisors regarding individuals receiving services in areas such as cognitive/perceptual skills, sensorimotor function, musculoskeletal status, and central nervous system status;
  • Integrate input from individuals receiving services, their families, and direct care staff into treatment planning;
  • Document treatment outcomes and progress in a clear and concise manner;
  • Provide training to individuals receiving services, their families, and direct care staff on specific supportive programs;
  • Participate in care plan/team meetings as needed for individuals receiving occupational therapy services;
  • Implement treatment plans developed by occupational therapists, which may include range-of-motion exercises, therapeutic activities, cognitive/perceptual retraining, and adaptive equipment training;
  • Perform other duties as required to ensure the provision of high-quality care.
Requirements
  • Bachelor's or master's degree in occupational therapy from an accredited institution;
  • NYS Certified Occupational Therapy Assistant (COTA) license;
  • Desirable qualifications: experience working with individuals with intellectual and developmental disabilities, wheelchair assessments.
