Home Health Care Nurse

We are seeking a skilled and compassionate Home Health Care RN to join our team on a per diem basis.
Responsibilities:
- Deliver high-quality nursing services to clients in their homes, providing case management and support.
- Conduct initial assessments and develop plans of care for home care clients.
- Document medical information accurately, including OASIS assessments as required.
- Initiate preventive and rehabilitative nursing procedures, referring clients to community services as needed.
- Prepare and submit clinical and progress notes daily, ensuring quality and accuracy of patient records.
- Coordinate services, communicating effectively with physicians, management, and other personnel.
- Ensure compliance with Medicare and Mass Health standards.
