Job Summary

We are seeking a highly motivated and experienced Registered Nurse to join our Hospital at Home team at South Shore Health. As a key member of our team, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Responsibilities
  • Assess and triage patients in their homes, developing and implementing individualized plans of care.
  • Provide safe and high-quality care for acutely ill patients, working collaboratively with the interdisciplinary team.
  • Conduct eligibility screening of patients, obtain verbal consent, and facilitate transfer to the Hospital at Home unit.
  • Respond efficiently to patient calls and messages, providing compassionate and timely support.
  • Develop and implement patient education plans, ensuring patients and families are empowered to manage their care.
  • Collaborate with the interdisciplinary team to promote safe discharge plans and optimize patient outcomes.
  • Document all work in the health record, adhering to unit and hospital guidelines and policies.
Requirements
  • Graduate of an accredited School of Nursing, with a Bachelor's Degree preferred.
  • Minimum of 2 years experience in acute or emergency settings, with experience in chronic/complex disease management or home care a plus.
  • RN - Registered Nurse license in the state of Massachusetts.
  • Basic Life Support (BLS) Certification.
Preferred Qualifications
  • Experience with Epic and other electronic health record systems.
  • Ability to navigate multiple technology platforms, including mobile applications.
  • Strong interpersonal and communication skills, with a focus on customer service.
Work Environment

This role requires frequent standing, walking, and lifting, with occasional driving required. The ability to work in a fast-paced environment and adapt to changing circumstances is essential.
