**Job Overview**

H2 Health Prime Living is a leading provider of care services in senior living communities. We are seeking a qualified Physical Therapist Assistant to join our team.

As a Geriatric Care Specialist, you will work closely with our experienced clinicians to deliver high-quality patient care and achieve exceptional outcomes.

**Competitive Compensation:** $70,000 - $90,000 per year

We offer a comprehensive compensation package that includes:

  • Base salary: $70,000 - $90,000 per year
  • Performance bonus program

**Job Responsibilities:**

  • Provide 1:1 patient care in assisted living and memory care settings
  • Treat patients with a range of orthopedic conditions, neurologic movement disorders, post-op cases, and balance programs

**Required Skills and Qualifications:**

  • Graduate of an accredited Physical Therapist or Physical Therapist Assistant program
  • Prior experience preferred, particularly in memory care
  • Active or eligible state license to practice
