Nurse Care Manager

We are seeking a skilled and compassionate Registered Nurse to join our team at Ciena Healthcare. As a Registered Nurse, you will play a vital role in providing high-quality patient care and leading our nursing team.
About the Role:The Registered Nurse will be responsible for planning, coordinating, and providing nursing care to our residents. This includes assessing their health needs, developing individualized care plans, and implementing evidence-based interventions to promote optimal health outcomes.
Responsibilities:- Supervise and coordinate the care provided by LPNs, CNAs/STNAs, and other team members
- Provide safe and accurate medication-related interventions to residents
- Assess resident health and notify physicians of changes in status, promptly implementing new orders
- Develop and implement individualized care plans based on resident assessments
- Contribute to resident assessments (MDS/CAA's) and the development of care plans
- 1-3 years of experience in a long-term care setting preferred
- Current Registered Nurse (RN) licensure in the state
- CPR certification
