The Ambulatory Care Nurse RoleThe Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to assigned populations. Working collaboratively with physicians, the ACN identifies at-risk patient populations who require care management services.
The ACN serves as a patient/family advocate, acting as a communication link between the physician, patient, family, and interdisciplinary healthcare team, as well as community resources. The ACN supports the mission, values, and management of patient care services at WakeMed.
Responsibilities- Provide nursing care management services to assigned populations
- Collaborate with physicians to identify at-risk patient populations
- Serve as a patient/family advocate and communication link
- Support the mission, values, and management of patient care services
- Diploma in Nursing or Associate's Degree in Nursing
- Registered Nurse licensure
- Monday to Friday, 8am-4:30pm schedule
