Job Title: Weekend Care Coordinator RN
About Liberty Health: At Liberty Health, we prioritize the unique needs of our patients and families facing terminal illness. Our state-of-the-art care and pain management services are delivered by specially trained staff who emphasize strength, dignity, and compassion.
Salary: $10,000 Sign on Bonus plus a competitive salary of approximately $80,000 per year (based on 2 years of experience).
Job Responsibilities:
- Care for and document patient care that meets reimbursement guidelines.
- Coordinate physical care with the patient, family, physician, and Hospice team.
- Attend and participate in IDT meetings and quality review as scheduled.
Required Skills and Qualifications:
- Current RN licensure in the state of practice.
- Successful completion of a nursing education from an approved school of nursing.
- At least 2 years of clinical nursing experience, with at least 1 year of hospice nursing experience preferred.
