Job Summary

We are seeking a highly skilled and experienced Nurse II to join our team at the Texas Department of Aging & Disability Services. As a Nurse II, you will play a critical role in providing exceptional patient care and leading our healthcare team.

Key Responsibilities
  • Assess and Plan Patient Care: Conduct thorough assessments of patients' health and mental status, and develop and implement individualized care plans to meet their unique needs.
  • Supervise and Coordinate Staff: Provide guidance and oversight to nursing staff, ensuring that they have the necessary resources and support to deliver high-quality patient care.
  • Collaborate with Interdisciplinary Teams: Work closely with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Monitor and Evaluate Patient Outcomes: Track and analyze patient outcomes, identifying areas for improvement and implementing changes to enhance care.
  • Communicate Effectively: Develop and maintain effective relationships with patients, families, and healthcare teams, ensuring that all stakeholders are informed and engaged in the care process.
Requirements
  • Knowledge and Skills: Possess a strong understanding of nursing standards, policies, and procedures, as well as excellent communication and leadership skills.
  • Experience and Education: Hold a current RN license and have a minimum of 2 years of experience in a healthcare setting, with a focus on patient care and leadership.
