Position Title: Support Coordinator

Reporting To: Lead Support Coordinator

Role Overview

The Support Coordinator for NYAP's La Jornada (The Journey) Program, focusing on Long-Term Foster Care (LTFC), plays a crucial role in positively influencing the lives of minors who have arrived in the U.S. without a parent or legal guardian. This position involves close collaboration with minors, foster families, sponsor families, and community partners to provide advocacy and support towards achieving specific goals.

Key Responsibilities

The Support Coordinator will undertake various tasks, including but not limited to:
  • Conducting all work in alignment with the mission, values, and principles of the National Youth Advocate Program.
  • Managing the intake, placement, and discharge processes, including conducting and updating assessments, service plans, and necessary documentation.
  • Writing and submitting required progress notes and associated documents in a timely manner.
  • Collaborating with the team to identify primary sponsors for minors and conducting necessary assessments.
  • Performing case reviews and maintaining regular contact with families through planned and unannounced visits.
  • Traveling to foster family and sponsor homes for assessments and home visits.
  • Preparing for and participating in administrative and legal case reviews.
  • Communicating with potential foster families and sponsors to gather necessary information.
  • Responding to after-hours emergencies as needed.
  • Participating in weekly supervision meetings and engaging in the supervision process.
  • Maintaining organized family case records in compliance with relevant regulations.
  • Completing additional duties as assigned.
Minimum Qualifications
  • Bachelor's degree in education, psychology, sociology, or a related behavioral science field.
  • Experience in delivering social services to diverse populations, particularly minors.
  • Bilingual proficiency in English and Spanish is essential.
  • Ability to organize and facilitate travel for youth, including air and ground transportation.
  • Willingness to work flexible hours to accommodate the needs of families and youth.
  • Capacity to maintain professionalism and respect for individuals from various backgrounds.
  • Strong advocacy skills for children facing various challenges.
  • Ability to work independently and collaboratively as part of a team.
  • Exceptional customer service skills focused on community connection and support.
  • Valid driver's license, reliable transportation, and appropriate insurance coverage.
  • Proficient in using technology, including computers and various software applications.
