Nurse Care Coordinator

We are seeking a skilled Nurse Case Manager to facilitate post-acute care and services for patients in Medical, Surgical, Pediatric, and Obstetrical Services at Mass General Brigham. This role requires effective coordination with the healthcare team, insurers, and community-based organizations to ensure seamless transitions and optimal patient outcomes.
About the Role
The ideal candidate will have prior experience in acute care hospital settings, healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery. They should be proficient in utilizing nationally accepted criteria for medical necessity and level of care. Strong communication skills and the ability to work independently with minimal supervision are essential.
This is a 32-hour per week position working 8-hour shifts during the day. We offer competitive compensation and a comprehensive benefits package, including medical, dental, vision, life insurance, and employee assistance programs.
Responsibilities
- Facilitate discharge plans for patients in Medical, Surgical, Pediatric, and Obstetrical Services, ensuring timely and safe transitions to post-acute care or home settings.
- Coordinate with the healthcare team, insurers, and community-based organizations to identify resources and services necessary for each patient's unique needs.
- Develop and implement individualized care plans that prioritize patient-centered outcomes and address potential barriers to care.
- Cultivate strong relationships with physicians, nurses, and other healthcare professionals to ensure collaborative and efficient care coordination.
- Monitor patients' health records and communicate regularly with the healthcare team to ensure continued appropriate level of care and compliance with Federal and State regulations.
Requirements
- Bachelor of Science in Nursing (BSN) degree from an accredited school of nursing.
- Current licensure as a Registered Nurse in the Commonwealth of Massachusetts, with a minimum of three years' clinical experience.
- Prior experience in case management, care coordination, and transitions of care preferred.
- Knowledge of healthcare reimbursement, quality improvement, and enhanced models of healthcare delivery.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Basic computer skills, including knowledge of Microsoft products and data entry abilities.
Salary Information
The estimated annual salary range for this position is $93,400-$123,500, depending on experience. Please note that this estimate may vary based on individual circumstances.
