Job OverviewWe are seeking an experienced Home Health Clinical Care Coordinator to join our team. As a member of our interdisciplinary team, you will play a crucial role in providing exceptional care to our patients in various settings within the community.
Key Responsibilities:- Develop plans of care that meet the unique needs of each patient, prioritizing safety and optimal outcomes.
- Provide comprehensive nursing assessments, education, and treatment, ensuring timely and effective communication with the multidisciplinary care team.
- Coordinate patient discharges, utilizing community resources to support seamless transitions and enhanced patient experience.
- Manage patient schedules, adjusting as needed to ensure alignment with clinical care and program requirements.
- Maintain accurate and up-to-date electronic health records, reflecting changes in patient status and plan of care.
- Graduate of an approved School of Nursing, with current Massachusetts license as Registered Nurse.
- Minimum one year recent nursing experience in acute or emergency setting required, with home care experience preferred.
- Excellent interpersonal, customer service, and communication skills, with ability to work effectively with diverse populations.
- Strong organizational and time management skills, with ability to prioritize tasks and adapt to changing circumstances.
- Familiarity with various computer software applications and technologies, with willingness to learn new systems as needed.
- Competitive salary: $90,000 - $110,000 per annum, based on experience.
