**Join CenterWell Senior Primary Care as a Field-Based Nurse Practitioner**

We're looking for a highly skilled and compassionate nurse practitioner to join our team of healthcare professionals. As a field-based nurse practitioner, you'll be responsible for providing high-quality, patient-centered care to seniors in their homes.

The ideal candidate will have a strong background in primary care, with experience in house calls or similar settings. A valid state driver's license, clean driving record, and proof of auto insurance are required.

**Key Responsibilities:**

  • Provide comprehensive health assessments and develop individualized care plans.
  • Prescribe medication and provide treatment for acute and chronic conditions.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.

**Requirements:**

  • Valid state driver's license and clean driving record.
  • Proof of auto insurance.
  • Bachelor's degree in Nursing (BSN) or Master's degree in Science of Nursing (MSN).
  • Minimum 2 years of clinical experience.
  • Active/unrestricted state license, DEA license, and board certification.
  • Strong technical experience, including proficiency in electronic medical records (EMRs).
  • Excellent communication and interpersonal skills.

**Estimated Salary:** $130,000 - $170,000 per year.
